Most insurance plans require a member to pay a co-payment per office visit. Please be prepared to pay the co-payment at the time of your visit. Our office will file a claim with your insurance provider for the balance. Please have your most recent insurance cards with you at the time of your visit so that we may make all necessary changes to your insurance plan.
